This 41-year-old presented with small, asymmetric breasts — a smaller right breast, chest wall and nipple positioning asymmetry, and an inverted left nipple — with the asymmetry becoming more noticeable after pregnancy. She was a 34A wanting a modest C cup and asked whether augmentation alone might also improve the inverted nipple. She underwent bilateral augmentation with a smooth-surface silicone gel implant placed under the pectoral muscle through a lower breast crease incision, which dramatically improved the inverted nipple and nipple asymmetry along with achieving her goals for size and shape.
