What Are My Implant Options?
Implant Types
Material
Implants are all made of a silicone shell and may be filled with:
- Saline (saltwater): Fully adjustable and often can be inserted through a smaller incision.
- Silicone gel: Filled with soft silicone for a natural look and feel. Current silicone gel implants used are all cohesive gel and come in variable firmness for a custom use depending on clients’ tissue and desired outcome. These implants tend to provide a more natural look and feel.
Shape
Implants come in multiple diameters and projections. Round implants are available with different profiles, or projections, ranging from a low profile to a high profile. The specific profile is selected to complement your tissue to achieve the most natural outcome desired.
Smooth surface breast implants are considered the safest product on the global market and are not impacted in breast cancers. For your safety, we exclusively use smooth implants, as textured implants have been associated with a very rare type of lymphoma called BIA-ALCL (breast implant-associated anaplastic larger cell lymphoma). There are textured implants available in the US market that are “shaped”. These implants are associated with specific additional risk considerations that can impact appearance long-term.
Implant Position
Breast implants can be placed either above or below the pectoral muscle. Dr. Rudderman will make the best recommendation for you based on the volume and consistency of your breast tissue and your desired outcome.
- Subglandular (above the pectoral muscle): Somewhat less postoperative discomfort, similar recovery in return to work and exercise.
- Submuscular (below the pectoral muscle): Often a more natural contour in patients with smaller breasts, reduced risk of rippling; may be more convenient for mammograms.
There are variations of implant placement, including dual-plane and subfascial pockets. Most breast augmentations are performed using a variation of submuscular placement. There are clients who are better candidates for subglandular or subfascial implant placement depending on their anatomy and lifestyle. Dr. Rudderman will discuss the benefits and considerations that should be made during your consultation. Clients who are more extreme athletes may be better candidates for subglandular placement, and occasionally, clients with prior augmentation who experience too much breast movement during exercise or pectoral activation (called animation deformity) will have implant pocket revisions and subglandular placement (above the muscle).
Incision Placement
This is one of the most-asked-about aspects of any breast surgery. There are three incision options that are conventional for breast augmentation:
- Inframammary (crease under the breast): Most common and well-hidden
- Periareolar (around the areola): Scars along the edge of the areola
- Transaxillary (armpit): No scars on the breast
Dr. Rudderman most commonly uses the breast crease incision for implant placement. This incision tends to heal with the most predictable minimal scar. It allows for the most direct approach to the implant pocket, whether above or below the muscle and avoids dissection through the breast tissue, reducing long-term risks of damage and scar tissue. There are also additional a theoretic advantage of avoiding breast glandular tissue that may harbor microbes that have been implicated in capsular contracture.
The periareolar incision in many patients may be more visible than the inframammary incision. This approach requires additional dissection that impacts both breast glandular tissue and the pectoral muscle. There is a greater potential for impact on sensation change to the nipple, and there may be an increased risk of capsular contracture due to contact of the implant with breast tissue during insertion. This incision often can retract over time resulting in a visible depression and thinning at this insertion site and may complicate revision, replacement or removal in the future.
The transaxillary incision eliminates a scar on the breast, but it can be quite visible when the arms are raised in clothing and bathing suits, and this approach is implicated in a higher long-term risk of lateral breast implant displacement, which may require additional revision surgery due to discomfort and visual changes.

As you will have been put under anesthesia, you will need a responsible adult to drive you home and stay with you for at least 24 hours, and bring you in the day after surgery for your post-operative visit.
