Ruptured silicone implants with severe capsular contracture can go undetected until surgical removal reveals the extent of the damage. This 56-year-old was concerned about hardening, soreness, and overall health related to her 25-year-old silicone implants, and exam showed a hard, painful breast with visible shape change consistent with Baker Grade IV capsular contracture; bilateral rupture was found at the time of surgery. Implant removal with capsulectomy was performed through an inframammary incision, and the original 255cc silicone gel implants were removed. The outcome was a more natural look with modest droop and asymmetry in residual volume, and she was very pleased with the decision and overall appearance.
