This 58-year-old sought removal of breast implants placed 23 years earlier along with options for a lift. Exam findings showed significant asymmetry, with a firm right breast consistent with capsular contracture and notable thinning of the breast tissue — more pronounced on the right and particularly evident below the nipple-areolar area on both sides. Her 420cc saline implants were removed with total capsulectomy through an inframammary crease incision, and bilateral breast reshaping and lift surgery stabilized the nipple-areolar position while improving volume along the lower breast bilaterally. She returned to her normal activity in 3-4 weeks without complications.
