Breast explant surgery, or implant removal, is a deeply personal choice, but it is also a choice that every breast augmentation patient will need to face eventually. There are a number of reasons women choose to have their implants removed, and the reason you are getting yours out will determine the specifics of your surgery, including what happens once the implant is removed.

During breast implant removal surgery in Atlanta and Alpharetta, Dr. Rudderman removes the implant and the capsule, the tissue that naturally forms around the implant. Following the removal, he can either reshape your breast tissue with breast lift techniques to restore a more normal appearance or occasionally use additional grafting techniques to address minor residual asymmetry. Women seek breast implant removal for many reasons, including:

  • Lifestyle changes
  • Aesthetic preference changes
  • The shape of your breasts has changed due to weight changes, hormonal changes, or aging
  • Medical concerns, such as implant rupture, capsular contracture, or breast implant illness (BII)
  • Your concern that the current implants are too old and may have a greater risk of rupture.
  • No longer want implants and want removal while they are in good health to avoid surgery later in life

Treatment selection for breast implant removal surgery is complex and requires consideration of a variety of surgical techniques. Success depends on awareness and careful assessment, then treatment of multiple factors, including soft tissue characteristics, life events (age, pregnancy, or specific health conditions), activity requirements, assessment of symptoms, and personal desires for outcome.

Developing a plan is not trivial, particularly one that clearly describes potential risk and both short- and long-term considerations for results. Our approach includes critical evaluation of your anatomy, attention to detail, matching the technical aspect of surgery to the specifics of clinical findings, and your desires for the outcome.

Selection of the specific approach used requires experience, judgement, technical skill, and artistry. The choices made impact long-term shape, potential for additional surgery, and both appearance stability and clinical and financial future risks. The goal is to select an approach that provides a higher probability of a predictable long-term outcome.

At Dr. Rudderman Plastic Surgery, we value collaboration and partnership. You should plan for your consultation to take at least one hour. During this time, you will meet with Dr. Rudderman and other members of our team to situate and integrate yourself into our practice. A partnership with Dr. Rudderman is a collaborative relationship with the entire team.

Our goal at every consultation is to meet you, learn who you are, what lifestyle you lead, and your goals for not just surgery but the next phase of your life. Understanding the way your breasts impact your daily life, physically and emotionally, is an essential component of effective treatment planning. Once we have a complete picture of what you are looking for, we can tailor a treatment plan for long-term results and sustainable beauty.

During your consultation with Dr. Rudderman, you should expect:

  • A full exam of breast tissue, skin elasticity, and symmetry
  • Extensive education regarding your implant options and safety considerations
  • A discussion of your goals, lifestyle, and desired outcome
  • A personalized surgical plan that may include implant removal alone, implant removal with a breast lift alone, implant replacement, or implant replacement with a lift
  • Education about your condition, treatment options, risks and benefits, recovery, and long-term expectations

Dr. Rudderman specializes in breast surgeries and understands the artistry and surgical complexity that they demand. His advanced technique and delicate, precision surgical skills achieve the most natural-looking results possible.

Determining the reason you want your implants removed is essential to tailoring your treatment plan to your ideal outcome. Women who present for implant removal evaluation, in many cases, have health concerns. The general expectation for treatment includes implant removal and capsulectomy. Your body naturally responds to the implant, which is a foreign object, by developing tissue that surrounds the implant, called the capsule. Limiting surgery to the safest and least invasive method is often the best way to address your concerns and restore your health. That said, there are a few different options for implant removal surgery based on your needs and health concerns, including:

  • Capsulectomy with implant removal
  • Implant removal alone

The specific technique used for the treatment of the capsule when implants are removed should be carefully considered, and each patient should have access to the best educational approach available at the time of treatment.

Consensus Statement on Breast Implant Capsulectomy Definitions and Management Created by Breast Surgery Collaborative Community, The Aesthetic Society / The Aesthetic Foundation, June 8, 2024

“The FDA recently created a strategic priority for the creation of collaborative communities to bring together stakeholders in the medical device realm to solve challenges and achieve a collective betterment for patients, physicians, industry, as well as the medical community at large. The Breast Surgery Collaborative Community (BSCC), a consortium of patient advocates, board-certified plastic surgeons, the medical device industry, and others with a vested interest in enhancing breast surgery safety, was assembled to offer clarity for women seeking guidance on the removal of breast implants related to health concerns.”

“The BSCC consensus statement defines four types of capsulectomy:

  1. Total intact capsulectomy: Complete removal of the breast implant capsule as a single unit.
  2. Total capsulectomy (total precise capsulectomy): Complete removal of the breast implant capsule, not necessarily done as a single unit or in one piece.
  3. Partial capsulectomy: Removal of the breast implant capsule, where some capsule is left behind.
  4. En bloc capsulectomy: Removal of the breast implant capsule with a margin of uninvolved tissue for treatment of suspected or established breast implant-associated cancers after appropriate medical workup.”

The statement explicitly declares, “the absolute and only indication for an en bloc capsulectomy is for an established or suspected breast implant-associated cancer after appropriate medical workup.” This decision for surgery is clearly a part of the informed consent process, and any decisions should be shared between the patient and surgeon in a formal discussion around risk assessment, benefits, and alternatives, as well as the expected outcomes.” – The Aesthetic Societ,y June 8, 2024.

Many patients have adequate breast volume to maintain a normal appearance after implant removal. Some find that their breast volume has increased over time, particularly with weight changes, pregnancy, and menopause. A breast lift can help reshape and restore a more youthful breast look and can be associated with dramatic improvement in comfort.

Some women avoid having a breast lift after implant removal due to concerns about scarring and healing. It is not uncommon for a surgeon to advise you to keep your implants or have new breast implants inserted. Many women simply no longer want this option. During your consultation, Dr. Rudderman can assess your needs and recommend the best approach for achieving your goals. This may include the removal of extra skin and adjustments to the breast tissue, especially if the breasts may become droopier after explant surgery.

Breast lifts work well for women with little breast volume who have concerns they may be “traumatized” by the thought of removing implants when they feel the “only” breast tissue they have are the implants. A breast lift can effectively reduce skin laxity to “repackage” the remaining breast tissue, optimizing the volume in a “smaller” package to improve appearance. The remaining volume of the breasts can usually be reasonably estimated. There may be areas of asymmetry, wrinkling, or indentations after implant removal that cause concern. Many times, these irregularities improve during healing. In the few patients who require additional treatment, additional skin may be removed, tissue repositioned, and, in some cases, fat grafting may be performed as a secondary operation to address contour irregularities.

How Should I Prepare for Breast Implant Removal Surgery?

Preparing for breast implant removal surgery involves a few steps, but mostly, you should get excited about your upcoming transformation. Whether you are about to feel physically better or just more confident, implant removal is as transformative as getting your implants the first time. In general, you should:

  • Prioritize nutrition: It can be helpful to prepare balanced, nutritious meals or arrange for a meal delivery service. During recovery, getting optimal nutrition is essential to ensure your body has everything it needs to heal.
  • Plan for help: You may need to arrange childcare, pet care, or help around the house during your immediate recovery. Plan for 3 to 4 days of help with children and household tasks.
  • Medical review: Dr. Rudderman may recommend running routine medical tests to ensure you are healthy enough for surgery. These tests may include routine blood tests, an EKG, or a mammogram.
  • Consider your medication: During your consultation, Dr. Rudderman will review your medications, vitamins, and supplements. You may need to discontinue anything that increases the risk of bleeding or bruising for 2 weeks before and after surgery, including fish oil, garlic, ginseng, vitamin E, Ibuprofen, or other anti-inflammatory medications.
  • Quit smoking: Nicotine use negatively impacts healing and can increase your risk of complications. Dr. Rudderman recommends all patients discontinue nicotine use at least 4 weeks prior to and following surgery to help ensure optimal outcomes and the best possible recovery, and possibly longer, depending on the operation recommended.
  • Focus on a healthy lifestyle: Maintaining a healthy lifestyle is one of the best things you can do to prepare for any surgery. Develop or continue a sustainable exercise routine that will support a stable weight, long-term health, and a higher probability of a smooth recovery.
  • Drink plenty of water: Staying hydrated is essential to your recovery. Drink plenty of water during the days leading up to your surgery to support recovery.
  • Get excited: Your breast asymmetry surgery is an investment in your quality of life. Get excited about who you will be, what you will wear, and what you will get to do once you have fully recovered. Reclaiming your body and self-confidence is always something to be proud of.

How Is Breast Implant Removal Surgery in Alpharetta, GA Performed?

All breast implant removal procedures take place in a fully accredited surgical center with state-of-the-art technology, hospital-level safety standards, and an experienced medical team dedicated to your care. Additionally, Dr. Rudderman takes pride in performing every single aspect of your surgery himself. As you go into surgery, you can feel confident knowing you are being cared for by the most qualified professionals available at every step of the process.

  • 1. Day of Surgery Prep: On the day of your surgery, you will arrive at the surgical center and meet with Dr. Rudderman to review your treatment plan and ask any last-minute questions you may have. He will then mark your breasts for surgery, and you will meet with a board-certified anesthesiologist.
  • 2. Incisions: When you are sedated and prepped in the surgical suite, Dr. Rudderman will make the appropriate incisions along the crease for implant removal ( and capsular removal ).
Incisions for a lift/reshaping surgery will often include around the areolar margin to reposition and downsize the areola, and vertically to access the underlying breast tissue and to help restore volume to the central lower breast in addition to the incision along the crease (inframammary breast fold, where the lower breast attaches to the chest).
  • 3. Reshaping: Once he has access, Dr. Rudderman will remove the capsule and implant. For many, he will perform a modified breast lift to achieve a natural, proportional look. Depending on the tissue condition, specific circumstances, and history, for some, to more safely and predictably improve surgical outcomes, he may recommend that women fully heal from implant removal before the breast lift portion of their treatment plan.
  • 4. Incisions: After surgery is complete, Dr. Rudderman will close your incisions with sutures, apply a light surgical dressing, and fit you with a soft bra to support your incisions and your new breasts. Dr. Rudderman’s delicate approach to tissue care and suturing helps promote optimal healing of the breast tissue and incisions.

Most breast implant removal procedures take 1 to 3 hours and are performed on an outpatient basis.

The operation and artistry involved in breast implant removal procedures are specific to each woman. During your consultation, you will receive extensive education regarding your condition, your concerns, your options, your risks and benefits, and your probable outcome. The goal of every procedure is a safe surgery, excellent appearance, normal anatomic redistribution, and an outcome that lasts for years and ages appropriately.

What Should I Expect After Explant Surgery in Atlanta, GA?

Immediately after surgery, you will be wearing a surgical dressing and a bra. As you will have been put under anesthesia, you will need a responsible adult to drive you home and stay with you for at least 24 hours. In general, you can expect the following during your Atlanta breast explant recovery:

Physical Recovery

  • You can expect mild swelling and some discomfort. Dr. Rudderman will prescribe pain medication for comfort management, but this should resolve on its own. After 24 to 48 hours, you should be able to manage discomfort with over-the-counter pain relievers like acetaminophen.
  • Swelling is generally mild and begins to resolve on its own in 2 to 3 weeks.
  • Sutures are absorbable and do not need to be removed.
  • You will have brown paper tape on the incisions to protect the area and help with minimal scar formation.
  • You may have a drain tube placed that needs to be checked several times a day. This drain is commonly removed 24 hours later during your first follow-up visit.

Return to Normal Activities

  • Most patients can return to desk work in 3 to 4 days. If your job requires physical activity, you may need to take more time off or request modified duties while you recover.
  • Women with children often can return to full childcare within reason in 3 to 4 days, including carpool and driving.
  • Avoid aerobic exercise for 3 weeks or until Dr. Rudderman has cleared you to do so. Most clients may begin more aggressive physical activity at 4 weeks, including yoga, tennis, swimming, boot camp, and Pilates. We will help guide your return to activity at each post-operative visit.

Post-Surgical Follow-up Visits

  • You will be seen in the office 24 hours post-surgery and given a more comfortable bra.
  • Regular follow-up appointments will be scheduled for:
    • 1 week post-op
    • 2 weeks later (3 weeks post-op)
    • 1 month later (7-8 weeks post-op)
    • 3 months later (6 months post-op)

Follow-up appointments are essential to ensure you’re healing as expected and are satisfied with your natural, age-appropriate results.

When Will I See My Breast Implant Removal Results?

Generally, you will notice an immediate difference in your breast appearance following surgery. Additionally, if you were experiencing symptoms of breast implant illness or pain associated with capsular contracture, many clients will notice relief from their symptoms soon after removal.

A brief overview of a realistic timeline of results is as follows:

  1. You will notice an immediate change in your breast appearance following surgery.
  2. By 6 weeks, most of the swelling will be gone, allowing you to have a better idea of what your final result will look like.

Atlanta Breast Implant Removal FAQs

Search our list of FAQs for the answers you are looking for.

Changes in Breast Shape Over Time

Weight fluctuation, pregnancy, menopause, and hormonal changes can alter breast shape and symmetry. These shifts may make breasts appear unbalanced or fuller than desired. Explant surgery can restore a natural breast contour, and many women are pleasantly surprised at how youthful and proportionate their natural breast shape appears after implant removal.

During your consultation, Dr. Rudderman will assess implant position, tissue characteristics, and expected volume changes. In some cases, a breast lift (mastopexy) is recommended at the time of removal to maintain a youthful contour.

Implant Rupture or Leakage

Breast implants can rupture due to wear, aging, or trauma.

  • Silicone implant rupture can be difficult to detect because the gel may stay contained within the capsule. Symptoms may include firmness, discomfort, or a change in shape.
  • Saline implant rupture is easily visible — the breast deflates as the saline is safely absorbed by the body.

Common signs of implant rupture include:

  • Change in breast size or shape
  • Pain, tightness, or discomfort
  • Hardening or lumps under the skin
  • Inflammation or swelling

If calcification or capsule thickening is found, Dr. Rudderman may perform a capsulectomy (removal of the capsule tissue) and send samples to pathology for evaluation.

Capsular Contracture (Breast Hardening)

A natural capsule forms around all implants as part of the body’s response to the implant. In some women, the capsule thickens and contracts, causing the breast to feel firm, distorted, or painful.

Symptoms of capsular contracture:

  • Firmness or tightness of the breast
  • Pain or soreness
  • Misshapen or asymmetrical appearance
  • Difficulty wearing clothing or exercising comfortably

Capsular contracture can occur with saline or silicone implants, textured or smooth, and above- or below-muscle placement. Treatment often includes total capsulectomy to remove the hardened capsule and restore soft, natural tissue.

Breast Implant Illness (BII)

Some women develop systemic symptoms they believe are related to their implants. This condition, called Breast Implant Illness (BII), can include:

  • Persistent fatigue
  • Brain fog or memory issues
  • Heart palpitations
  • Hair loss
  • Joint or muscle pain
  • Skin rashes or inflammation
  • Digestive issues
  • Anxiety, depression, and mood changes
  • Sleep problems or mood changes

Although scientific studies have not established a single cause, many women experience significant symptom improvement after implant removal. Dr. Rudderman provides compassionate, evidence-based care and coordinates with your medical team to support overall wellness.

All capsules removed during surgery are sent to the Northside Hospital Pathology department for microscopic evaluation, and a report is generated for your medical record with findings noted. It is very rare for any neoplasm (cancer) to be related to capsule removal.

Your outcome depends on how much of your breast volume is natural tissue. Many women are pleasantly surprised by how balanced and natural they look after removal — and options such as a lift or grafting can enhance the result.
