Breast implant illness symptoms — palpitations, shortness of breath, fatigue, brain fog — can prompt a patient to consider removal even when bloodwork comes back normal. This 51-year-old had a 20-year history of breast augmentation with 270cc saline-filled smooth implants when she developed these symptoms. On exam she had modest asymmetry of breast volume and nipple position, with some flattening of the lower breast on the left side, though she had otherwise good volume of tissue above the implants. Implant removal and capsulectomy were performed through a lower breast crease incision, chosen specifically to keep the resulting scar less visible. She healed well, with resolution of the majority of her prior symptoms.
